Please register your patient information on our Patient Portal or you may print off our forms by clicking each form on the left of the screen, fill out the forms and bring them with you to your appointment. You can use your patient portal account to send a secure message to your doctor, view your medical records, and pay your bill online.

Please arrive 30 minutes early to your first visit in order to allow us time to process your insurance and new patient information. Please bring your photo ID, insurance card(s) and co-payment or co-insurance if you have one.

You may also want to contact your insurance company to ensure we are in-network with your insurance plan.

We ARE in-network with Medicare, Missouri Medicaid plans such as Straight Medicaid, Aetna Better Health, Home State, and Well Care. In addition to those plans we are also in-network with most commercial insurance plans that St. Luke’s Hospital accepts to provide convenient care to our patients that require out-of-office procedures.

Our office will be accepting Humana Medicare PPO, HMO, POS, and Private Fee-For-Service Plans as of November 15th, 2015.
